Do You Need Copies of Medical Records?

You may obtain copies of your medical records or a program attendance letter for your own use or have records sent elsewhere, but only at your written request. You must complete an Authorization to Release Protected Health Information and return it to us with your original signature. The form CANNOT be returned to us by fax. These additional measures are important to ensure the protection and security of your confidential health information. . There is no charge for a program attendance letter; however, you may be charged for copies of your medical records based on the number of pages copied. Request for Medical Records Process

  1. Open and print the Authorization to Release Protected Health Information form.
  2. Complete and sign the release form. Please note: The signature on the release must match the original signature in your medical record.
  3. Return the form with your original signature by U.S. Mail or other service such as Fedex. We do not accept faxed releases.
